Thoracolateral flap is a local flap for partial breast reconstruction. It allows volume replacement while preserving the shape of the breast. It is especially helpful for small breasts with limited ptosis, in which an oncoplastic mammaplasty is not appliable. In this case, however, the area of resection was too huge, up to one-third of the breast. The solution to preserving the breast was associating an immediate fat grafting into the flap and the remaining breast. The shape of the breast has been preserved, but a new session of lipofilling may be possible in the future according to the patient's will.
